We are seeking a dedicated and experienced Case Manager to join our team in Buffalo, NY. The ideal candidate will have a strong background in healthcare or homecare, with specific experience in managing insurance cases. This role involves working closely with elderly and disabled clients to ensure they adhere to their medical schedules and appointments.
Key Responsibilities:
- Manage and coordinate insurance cases, ensuring all client needs are met.
- Assist clients (elderly and disabled) in following up with their medical appointments and schedules.
- Act as a liaison between clients and insurance companies, ensuring smooth communication and resolution of any issues.
- Develop and implement care plans tailored to individual client needs.
- Maintain accurate and detailed records of client interactions and progress.
- Provide support and guidance to clients, helping them navigate through the healthcare system.
- Collaborate with healthcare providers, insurance representatives, and other stakeholders to ensure comprehensive care for clients.
Qualifications:
- Bachelor's degree in Healthcare, Social Work, or a related field preferred.
- 1-5 years of experience in a similar role, particularly in healthcare or homecare.
- Proven experience working with insurance companies and managing insurance cases.
- Strong organizational and time-management skills.
- Excellent communication and interpersonal skills.
