We are seeking a dependable Case Manager to join our team. This position is designed to improve or maintain a member’s physical, emotional, and economic independence in the community with or without support.
Responsibilities:
- Conducting weekly visits to all members on your caseload
- Ensuring all essential furnishing, utilities, and community services are in place for new discharges
- Documenting all client encounters in MedCompass
- Identifying and addressing potential risk areas in client’s well-being
- Responding to client questions or concerns
- Communicating with Case Managers to coordinate services or concerns of client
- Helping members obtain income, food stamps, utility assistance, etc.
- Creating Risk Mitigation plans for new members
- Monitoring member’s units and requesting increases as needed
- Creating client Care Plans
- Attending weekly staff meetings
Qualifications:
- Degree in Human Services or related field preferred but not required. Depending on work experience
- Must have reliable transportation and a valid license
