Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have experience in mentoring, behavioral health, social work, and crisis management. This position offers the opportunity to make a positive impact on individuals in need.
Duties:
- Provide case management services to clients in need of support
- Conduct assessments to determine clients' needs and develop individualized care plans
- Coordinate and monitor services for clients, ensuring they receive appropriate resources
- Supervise and support clients in various settings such as care facilities
- Advocate for clients' rights and access to services
- Collaborate with other professionals including social workers, addiction counselors, and healthcare providers
- Maintain accurate and up-to-date client records
Requirements:
-Must pass Guardianship end of course exam
- Bachelor's degree in Social Work or significant working experience
- Experience working in case management
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
-Excellent time management and organizational skills
-Ability to multitask and problem solve with minimum supervision
