Kendal at Hanover is seeking a Community Clinical Care Coordinator (CCCC). The CCCC is responsible for promoting physical, psychosocial, and spiritual health and wellness for Kendal at Hanover residents in various levels of care with emphasis on supporting appropriate admissions, transitions, and placements within the community. The CCCC is responsible for resident care coordination including potential issues pertaining to appropriate resident placement within the community, availability of services, or other safety concerns and works toward a resolution through collaboration and transparency. This role will partner with various departments in ensuring needed services are provided with the scope of the setting and appropriate communications and operational plans for residents’ safety, health and wellness are developed and remain in line with person-centered service offerings and resident choices. To aid in coordination of resident needs and services, the CCCC will oversee the homemaker staff and transportation team. Oversight of homemaker services includes intake, scheduling, and billing management. Oversight of transportation includes assistance with scheduling, billing management, and appointment follow up with the clinical team as indicated.
Essential Functions
- Identifies clinical needs and assists in coordinating services and placement for Independent Living residents at time of application, transition, change in status, or crisis. Establishes close relationships with and serves as primary contact for health and wellness needs for Independent Living residents. Partners with various departments as necessary to ensure appropriate measures are taken to ensure resident health, wellness, and safety needs are met through offered services and appropriate transitions when required.
- Commitment to prioritizing person-centered care and minimizing barriers by identifying critical resources and services to meet resident needs, helping residents at all levels navigate the continuum of care, coordinating clinical services through various health care systems, and promoting resident health and wellness while communicating with transparency, honesty, and empathy.
- Monitoring and coordination of resident care needs. Works closely and communicates regularly with all members of the interdisciplinary team to identify resident needs and changes, remove barriers and minimize risk, ensure safe and timely transitions, communicate effectively with the resident and their support team, assist in resident crises, and provide the highest quality of services to residents.
- Works closely with the Community Care Coordinator , the Dartmouth Health Clinic and consulting providers, as well as members of KaH’s clinical team, to ensure positive clinical outcomes for residents, minimize anxiety and disruption when transitions are indicated, and coordinate timely follow up on medical, psychosocial, or safety recommendations.
- Meets with residents and/or resident representatives upon entrance to KaH and as needed based on resident needs or changes to discuss health and wellness assessments, provider recommendations, and review other care aspects as needed. Works with resident and/or resident representatives to offer choices and ensure the resident is aware of clinical choices both on and off campus. Connects resident and/or resident representatives with additional healthcare resources as needed.
- Works closely with the marketing department to appropriately promote and advertise current health and clinical services for potential and current residents. Assists with resident interviews and assessments for prospective residents at all care levels. Establishes realistic expectations and communicates KaH’s structure and abilities at early stages to maintain resident trust and ensure understanding of KaH’s structure, abilities, and limitations.
- Fosters and strengths health and wellness initiatives and collaborations. Advocates for existing resident services while delivering ongoing health updates and educational sessions on clinical offerings and organizational structure.
- Stays current on latest industry health trends, standards of practice, and regulatory requirements. Selects health assessment tools to be utilized and reviews them for accuracy and compliance while ensuring they meet the needs of the community. Works to revise forms and processes as needed to ensure efficient and effective practice.
- .Supervises homemakers to ensure service agreements are honored and services are delivered appropriately and within their scope of practice. Supervises transportation services staff to ensure that transportation services meet residents' medical transportation needs in a timely and appropriate manner. Ensures that effective communication regarding eclinical follow-up for residents occurs.
- Identifies steps to be taken to meet short- and long-term goals and strategic objectives; coaches and develops staff and participates actively within the larger KaH and outside community as needed and or assigned.
Education and Experience
- Registered Nurse or Licensed Practical Nursing Licensure from the State of New Hampshire Board of Nursing or other Compact State.
- Experience working in a leadership role with responsibilities including supervision, scheduling, and resource appropriation within a Continuing Care Retirement Community, Assisted Living, Nursing Home or Community Case Management position preferred.
- Experience working as or with Social Worker preferred. Experience working with residents and their family/support systems through challenging situations or changes in status preferred.
- Financial Management skills; business acumen; familiarity with business practice and principles preferred but not required.
- Ability to direct resident care and services in geriatrics which meets or exceeds facility quality and service standards.
