Position Overview:
We are seeking a dynamic, creative, and compassionate Care Coordinator to join our team. As a pivotal figure, the Care Coordinator will ensure our clients receive exceptional care and unparalleled satisfaction. This role encompasses coordinating all aspects of caregiver training, conducting thorough client assessments and reassessments, and providing ongoing support to both clients and caregivers.
The Care Coordinator will be responsible for developing meticulous care plans and collaborating closely with Scheduling Managers to align caregiver assignments with client needs, preferences, and caregiver qualifications. Additionally, fostering revenue growth by cultivating and maintaining relationships with referral sources will be a key focus.
The Care Coordinator will actively facilitate the delivery of in-home care and support services to clients in Yolo and Solano Counties, requiring travel within the specified area. By embracing this comprehensive role, the Care Coordinator will champion the highest standards of care delivery, client satisfaction, and operational efficiency within our organization.
Responsibilities:
- Coordinate all caregiver training, from orientation and throughout their employment.
- Conduct client reassessments to ensure the ongoing effectiveness of care plans.
- Conduct client inquiries and assessments with an 80%+ close rate
- Assist with HR functions, including managing certifications and separations.
- Provide world-class customer service by assisting with phone volume and addressing client inquiries.
- Cover open client shifts as needed to maintain continuity of care.
- Assist clients in determining the type and duration of services needed to meet their specific requirements.
- Provide continuity of care for clients during and after referral.
- Produce high-quality care plans and work with Scheduling Managers to ensure shifts and hours are matched with Caregiver qualifications, preferences, and availability to client needs
- Drive revenue growth by developing and maintaining relationships with referral sources
Qualifications:
- Minimum of 1 year of related experience (meeting and signing clients, interfacing with care partners) in the senior care industry (home care, home health, hospice, senior living facilities) with a demonstrated history of 80%+ close rate.
- Strong relationship-building skills with people from diverse and varied backgrounds and education levels.
- Strong interpersonal and communication skills.
- Excellent organizational and time management abilities.
- Ability to work independently and as part of a team.
- Proficiency and strong computer literacy in Google Workspace, WellSky, and other relevant software.
- Valid driver's license, auto insurance, and clean driving record.
- HCA # clearance and ability to pass DOJ/FBI livescan clearance.
- Previous sales experience is a plus.

Pay: $28.00 - $32.00 per hour
