Shallowford Wellness Center's Partial Hospitalization program is looking for a full time LISCENSED substance abuse and mental health therapist, LPC, LSWC, LMFT to provide clinical treatment in our mental health and substance abuse outpatient program.
Responsibilities:
- Attend staff meetings and inform staff of any important participant information.
- Implement recovery process for participants as defined by Shallowford Wellness Center's Partial Hospitalization Program goals and philosophy.
- Provide continuity of care as defined by treatment plan for each participant.
- Lead group therapy sessions daily.
- Conduct individual therapy sessions with each participant on caseload.
- Work collaboratively with the clinical team
- Coordinate and collaborate with off-site, community-based providers.
- Alert clinical team to any current or potential behavioral problems of participants.
- Coordinate care and conduct Utilization Reviews with insurers.
- Meet or exceed program productivity requirements.
Treatment Plans:
- Devise, with participant and treatment team, within 15 days, a comprehensive treatment plan based on Psychological Evaluation and other psychological assessments.
- Complete biopsychosocial assessment within 3 days of admission.
- Update treatment plans.
Progress Reports:
- Maintain continuous record of all contacts, events, and progress toward accomplishment of goals as stated in Treatment Plan.
- Record individual and group progress notes in the DDAP format after each therapeutic activity.
- Complete reviews in compliance with state licensing and other program guidelines.
- Maintain charts that meet all standards.
Continuing Care Plan:
- Assist treatment team and participants with the information of continuing care plans
- Other duties as assigned.
Qualification:
- Master’s degree and two years related experience with one of the following credentials: licensed in GA as a Professional Counselor (LPC), Licensed Social Worker (LSW), Clinical Social Worker (LCSW), Marriage and Family Therapist (LMFT) or Psychologist.
- Competence in individual, group therapy, and family intervention.
- Experience and training in evidence-based approaches.
- Ability to use therapeutic skills, which will assist consumers in process of recovery.
- Bilingual in Spanish preferred
