Title: Bilingual Care Coordinator

Summary: Under the general guidance of the Program Manager, this Bilingual Care Coordinator, Bilingual-Spanish required position, oversees the oral health care services to students who are part of the Chicago Department of Public Health’s School-Based Oral Health Program. This position will be involved with the oversight and coordination relating to integrated oral health care access, development of individualized care planning, care coordination, case review with providers, data entry, and interacting with community partners.
Essential Functions:
Duties & Responsibilities

Provides care coordination outreach to participants and their families, , providing answers and community referrals for participants as needed, including medical, oral health, public benefits, and others.

Manages assigned participant caseload through community outreach, conducting care coordination assessments and creation of related care plans, including social and cultural factors that influence health.

Participates in periodic evaluative reviews and in-house and external staff training to ensure that they understand professional standards and continue to adhere to such standards. Initiates requests for assistance or guidance from supervisor to address new issues or complex concepts affecting adherence to professional standards.

Monitors referrals for participants to partnering community hospitals and Federally Qualified Health Centers through professional communicative processes and other coordinated care interfaces; following up with participants and making appointments.

Actively participates in care planning that specifies oral health care coordination by referring to direct care resources to meet physical and psychosocial needs; by prioritizing problems and establishing mutually-agreed upon goals specific to the stage of participant activation; by using program planning and group concepts to meet the health needs of the special populations served.

Documents treatment plans and other vital information in electronic medical record after providing coordinated oral health services and intakes at the Pilsen Center for Oral Health Promotion.

Collaborates with other multidisciplinary professionals and community agencies to provide a continuum of coordinated care addressing health and related social determinants.

Participates as assigned in participant education including development of materials, conducting presentations or supporting other team members in such efforts.

Documents comprehensive, accurate, and continual data on client records and program reports.

Performs other duties as assigned.
Education and Experience:

High school diploma or equivalent and minimum of five years of experience in public health, case management, human services, or related field, or bachelor’s degree and one-year related experience in a public health, case work, human services, or relations field (such as health education, prevention, case management).

At least two years related experience working with vulnerable populations and or Medicaid recipients required.

Previous work in community-based, public health or managed care is preferred.
Knowledge/Skills/Abilities (K/S/A):

Ability to calmly and politely interact with participants to accurately define their question or concern.

Demonstrated computer proficiency, including knowledge of word processing, spreadsheet and database software. Experience with Microsoft Excel required.

Fluency in Microsoft Office especially Word and Excel; Ability to maintain and organize large quantities of information in an electronic database.

Bilingual Spanish-English required, verbal and written. Ability to write reports, correspondence, and instructions. Ability to effectively present information and respond to questions from staff, managers, participants, vendors, and the general public.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Attention to detail and accuracy to competently and effectively communicate orally and in writing for a variety of audiences; Ability to manage multiple tasks efficiently.

Ability to maintain client confidentiality, account for client monies, and avoid conflicts of interest in accordance with organizational policies and ethical standards required of the legal profession and the code of professional responsibility.

Ability to work independently and remain self-motivated with minimal direct supervision.
Work Arrangement:
This role is eligible for a “hybrid” or “remote” work arrangement. If hybrid, it is expected this person would be able to meet for “in person” meetings when needed or at the request of others. This role requires the person to attend three days a week to our Pilsen Center for Oral Health Promotion. In-person meetings may be required on a regular basis (weekly, monthly, etc.). There may also be the availability to work from home when appropriate.

Each program site operates on a 24/7 basis, 365 days per year. To meet both the needs of our participants and contractual obligations, employees are required to be flexible in regards to scheduling and work location.

Program sites are located throughout the Chicago area, including the North and South sides of Chicago, as well as the suburbs. Employees may be required to report to any one of our locations on a temporary or permanent basis.
