Compensation: $70,000 annual salary

Position Summary: The Supervisor of Case Management will lead and inspire a team consisting of Case Managers and an MSW student intern. A significant component of the team’s work is accepting referrals for homebound older adults, conducting in-depth assessments for new clients, developing tailored and comprehensive care plans, and authorizing services such as home-delivered meals and homecare services. The team makes referrals for additional services as needed, assists with the collection of documents and completes necessary paperwork for benefits.
The Supervisor should have an affinity for working with older adults, have strong interpersonal skills and work well with clients and colleagues. S/he should be a good communicator with strong written skills and attention to detail. The ideal candidate engenders trust, is committed to team building, embraces a style of communication that is clear, direct and kind, and is comfortable multi-tasking. The role calls for a self-aware, and effective leader with proven management and leadership skills.
Primary Responsibilities:
Participate in monthly meetings for CBN case management staff and subcontractors
Participate in weekly meetings for internal case management team
Participate in case conferences as an advocate for clients as needed
Provide weekly (and as needed) individual supervision for Case Managers and MSW student intern
Provide approximately 200 case management hours per year
Have the capacity to support and fill in for, as necessary, the Case Managers who manage a substantial case load and MSW student intern
Travel as necessary in order to meet with clients in their homes on the Upper East Side, East Harlem and Roosevelt Island
Provide expertise in, and guide staff towards appropriate resources for older adults based on thorough knowledge of relevant community resources
When a client is missing, support staff to take all reasonable steps to ensure that the client is safe
Support staff to refer to and ensure that APS (Adult Protective Services) follows up on cases where a client’s competency is questionable
In the event of emergencies, support staff to ensure that all their more vulnerable “red-coded” clients have someone who will check in on them
Support staff to secure foundation stipendiary funding allowing clients to remain in their own homes
Meet deadlines established by NYC Aging standards, including oversight of staff documentation and completion of assessments
Assist the Director and/or Assistant Director with maintaining detailed computerized statistical and narrative records for internal CBN staff
Initiate and maintain contact with representatives of the community including strategic partner organizations and government agencies on behalf of clients
Participate in agency-wide staff meetings, agency events, in-services trainings and conferences.
Attend the required number of trainings as outlined by NYC Aging standards
Stay up to date on relevant issues that relate to the field of aging
Accept other assignments, as appropriate, to meet the needs of the program and individual clients
Required Qualifications:
LMSW or LCSW with a minimum of 3 years of professional social work experience in aging services or related field and in supervising a team and/or informing clinical best practices
Advanced interpersonal and case management skills
Affinity for, and depth of experience in, working with an older adult population
Completed Seminar in Field Instruction (SIFI) training for supervising MSW students, if candidate does not have completed SIFI at time of hire it will be expected that they complete it within 1 year
Proven ability to lead case managers and MSW student intern, build individual capacities and promote excellence
Passion and sensitivity for CBN’s mission
Strong written and verbal communication skills
Ability to work pro-actively, both independently and collaboratively
Strong proficiency with database/information management systems and Microsoft Office Suite
