We are seeking a dedicated and knowledgeable Diabetes Educator to join our growing healthcare team. As a Diabetes Educator, you will play a vital role in empowering individuals with diabetes to manage their condition effectively, leading to improved health outcomes and quality of life. You will provide education, support, and guidance to patients, caregivers, and healthcare professionals to promote diabetes self-management and facilitate informed decision-making.

Job descriptions and needs would entail but not limited to:
- Conduct comprehensive assessments of patients' knowledge, understanding, and self-care practices related to diabetes.
- Develop individualized education plans tailored to patients' needs, preferences, and cultural backgrounds.
- Provide clear and concise explanations of diabetes pathophysiology, treatment options, medication management, blood glucose monitoring, nutrition, exercise, and lifestyle modifications.
- Offer practical strategies for coping with the psychosocial aspects of diabetes, including stress management and emotional well-being.
- Address common misconceptions and barriers to adherence to treatment plans.
- Foster a collaborative partnership with patients, empowering them to take an active role in their diabetes care.
- Collaborate with healthcare providers, including physicians, nurses, dietitians, pharmacists, and other members of the multidisciplinary team, to coordinate diabetes care plans.
- Participate in case conferences, team meetings, and quality improvement initiatives to optimize patient outcomes and promote continuity of care.
- Serve as a resource for healthcare professionals by providing evidence-based guidelines, protocols, and educational materials on diabetes management.
- Certified Diabetes Care and Education Specialist (CDCES) certification preferred.
- Current licensure or certification as applicable (e.g., Registered Nurse, Registered Dietitian, Licensed Clinical Social Worker).
- Solid understanding of diabetes pathophysiology, pharmacotherapy, nutrition therapy, and behavior change principles.
- Excellent communication, interpersonal, and teaching skills.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Proficiency in using technology for patient education and data management.

Pay: $35.00 - $46.00 per hour

- 8 hour shift
- Monday to Friday
