- The DSP is responsible for carrying out each individual's outcomes as described in the Individual Service Plan, as well as assuring that the individual's personal desires are carried out to the greatest extent possible. DSP's provide support to individuals in their day-to-day living in the home and in the community. The DSP assures that the Health and Safety of individuals is maintained and monitor for any changes in health status. DSPs communicate positively and professionally with individuals and their families. DSPs maintain all required documentation of work performed and outcomes completed.
- Assure that all outcomes are completed for each individual as described in the Individual's ISP. Provide assistance with ADL's as needed, such as but not limited to toileting, bathing, dressing, changing soiled briefs, shaving, brushing teeth, and grooming. Provide adequate nutrition by preparing and serving healthy meals and assisting with feeding as necessary. Maintain a clean and sanitary home environment by regularly sweeping, mopping, vacuuming, cleaning sinks, toilets, countertops and all other areas of the home. Provide assistance, including total assistance if needed, to individuals with domestic duties, such as laundry, making beds, and maintaining clean and orderly bedrooms. Coordinate and maintain personal schedule of individuals by providing transportation to all appointments (ex. medical, work, day support, community groups or churches) Create and maintain professional relationships with individuals, their families, case manager, staff members, and all other service providers. Document all work performed and outcomes completed according to licensing and company standards. Monitor individuals served for any changes in health status, such as but not limited to fatigue, lethargy, changes in appetite, rapid weight gain or loss, and any other significant changes. Notify the Group Home Manager immediately of any changes. Assist Individuals with taking medications according to the "5 rights" of medication management.
