Job Description
PURPOSE OF POSITION
Under general supervision of the Director of Social Work, a Social Worker Level II or higher, and the unit coordinator, the Social Worker Level I provides
SUMMARY OF DUTIES AND RESPONSIBILITIES
- Provides psychosocial assessment and housing evaluations on all homeless HASA clients Complete Psychiatric Evaluations for NYNY 2010e applications
- Upon obtaining LCSW licensure, conduct psychiatric evaluations for all HASA clients applying for NYNY Supportive Housing
- Completes all appropriate housing applications
- Assess the client's need for other mental health services and refer
- Provides crisis intervention as needed
- Notifies appropriate HASA staff when additional client related documentation is required in order to complete the housing application.
- Makes internal and external referrals as required/needed
- Documents all recommendations and completes all program related charting
- Maintains statistical data regarding interventions
- Completes all program related charting
- Responsible for covering the other HASA sites as needed
- Attend and participate in administrative and clinical meetings Present client case summaries for discussion and review. Supervises MSW students assigned by HRA HASA program
- Supervise staff
- Outreach to community resources.
- Interdisciplinary collaboration when necessary. Participation in regular on-going supervision. Make referrals to outside agencies.
- Field work as required
Minimum Qualifications
For Assignment to Level III
1. Valid license as an LCSW issued by the NYSED.

CERTIFICATION(S)/LICENSE(S): LCSW
EDUCATIONAL LEVEL: MSW
EQUIPMENT/MACHINES OPERATED:
Ability to operate typically office equipment; copier, fax, scanner, etc.
KNOWLEDGEABLE IN:
Preferred experience or education in health care, social services in an unserved community and working with individuals who have mental illness.
COMPUTER PROGRAMS/SOFTWARE OPERATED
Microsoft programs, Outlook. Experience with scheduling programs and electronic medical records helpful.
