Job Overview: We are seeking a detail-oriented Intake Coordinator to join our team caring for homeless veterans. The ideal candidate will have a background in veteran services, homelessness, and human services. Exceptional organizational skills are needed to manage patient intake processes effectively.
Duties:- · Client unit placement and bed management, Report bed census to VA, Explains general resident policies, rules, and fees during intake process, Outreach; encampments, Veteran’s Hall in Santa Cruz, using By Name List, contacting outside providers for potential referrals, Prescreen potential residents for eligibility and suitability for Grant Per Diem or Healthcare for Homeless Veterans programs. Manage resident database, HMIS entry/exits, CARS assessments, create reports, maintain CHSP data quality standards, Assigns new intake to case managers with Program Director collaboration, Community partnership engagement: CAT meetings with community providers to discuss homeless cases, manage the Veterans By Name List in collaboration with Nations Finest and VA. Interacts with government and outside agencies, Screen incoming mail for active clients/distribute to case managers, Participate in community events on behalf of VTC, Collaborate with VA liaisons on all matters affecting GPD and HCHV, Manage the GPD and HCHV intakes/beds in collaboration with the Program Director, Collaborate with all GPD case managers regarding clients and supports for new intakes, Ordering/shopping for veteran household supplies, Provide resources and referrals to walk-ins at Martinez Hall
