Job Overview:
We are seeking a dedicated Early Intervention Service Coordinator to act as the primary contact between families and service providers. The ideal candidate will guide families through the New York State Early Intervention program, ensuring they understand their rights and the array of services available to support their children's developmental needs.
Responsibilities:
- Serve as the primary point of contact for families, guiding them through the early intervention process.
- Assist families in planning and accessing the appropriate services for their children.
- Supervise, coordinate, and monitor the delivery of services, ensuring optimal use of resources.
- Participate in the development, review, and evaluation of Individualized Family Service Plans (IFSPs).
- Collaborate with families during IFSP meetings to detail the services provided.
- Maintain accurate records and documentation related to service coordination activities.
- Develop and manage caseloads, delegating and overseeing services to children and families.
- Engage in continuous education and attend staff in-services and meetings.
- Adhere to all regional early intervention policies and procedures.
Qualifications:
- Bachelor's degree in Psychology, Sociology, Social Work, Human Services, or related field.
- Comprehensive understanding of early intervention regulations and guidelines.
- Experience in an early intervention service coordinator role, preferably within NYC.
- Exceptional communication skills, both written and verbal.
- Ability to efficiently manage a multi-family caseload.
- Bilingual proficiency is highly desirable.
- Willingness to conduct visits in homes or community settings.
- A strong commitment to supporting and empowering families.
