Job Overview:
We are seeking a dedicated Memory Care Coordinator to join our team. The Memory Care Coordinator will report to the Health Service Director and will be responsible for overseeing and coordinating care services for individuals in need of assistance, ensuring their well-being and quality of life.
Responsibilities:
- Develop and implement care plans based on individual needs and preferences
- Coordinate with healthcare providers, family members, and other stakeholders to ensure comprehensive care
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Provide guidance and support to caregivers to ensure proper implementation of care plans
- Advocate for resident rights and ensure compliance with regulations and standards
- Assist in medication administration and management as required
- Collaborate with social workers, nurses, and other healthcare professionals to address complex care needs
- Specialize in areas such as home care, senior care, dementia care, or working with people with developmental disabilities
Skills:
- Proficiency in managing care services for individuals in nursing homes or community settings
- Knowledge of care planning processes and documentation requirements
- Experience in coordinating services for individuals with dementia or developmental disabilities
- Strong communication and interpersonal skills to interact effectively with residents, families, and healthcare providers
- Familiarity with medication administration protocols and best practices
- Understanding of social work principles and practices in a healthcare setting
