Job Summary:
Community Liaison staff are responsible for networking internally and externally with ACS, community partners, and other stakeholders. This position requires a unique individual who is detailed oriented and has strong verbal and written communication skills. The individual will be comfortable with identifying and pursuing creative problem-solving skills that are necessary for families who encounter the child welfare system, substance use, mental health, and domestic violence.
Conference Facilitators are responsible for helping to identify issues that may threaten the safety of children at home, address the status of behaviors or conditions that present risk of abuse or maltreatment to the child, and help family members create a plan that will address them while promoting the advancement of a permanency and/or stabilization plan for the child/family. Utilize a strength-based perspective in guiding the processes that is solution focused, and maintain a neutral and/or objective perspective while guiding the problem-solving solutions raised by staff, children, and/or family members.
Duties and Responsibilities:
Community Liaison and Advocate
· Dedicated 50% of the time to the following community outreach activities.
· Participate and conduct community outreach events.
· Build partnerships/relationships with external providers including but not limited to NYC schools public/private, Staten Island Mental Health, Richmond University Medical Center, Staten Island University Hospital, Family Justice Center, Courts, CHASI, Safe Horizon, Legal Aid, among other community organizations.
· Develop PowerPoint presentations and communication that will be distributed and communicated to service providers and families on Staten Island.
· Develop advocacy referral process and system for external organizations to refer community cases.
· Regularly attend and present the agency at community meetings and other events as it relates to the scope of the work.
· Work collaboratively with internal and external programs and departments.
· Attends all required trainings to enhance skills working with high-risk population.
· Complete other assignments as needed or designated by the Program Director.
Family Team Conference
· Responsible for participating in Clinical Diagnostic Team Meetings to ensure goals and outcomes identified for the service plan are incorporated appropriately.
· Responsible for ensuring the notification and coordination with ACS, FSU workers, Mental Health, Health, and Substance Use providers.
· Advocate and assist families with connecting to community-based resources/ organizations and making referrals to facilitate the services needed by the families.
· Participate in activities including initial intakes, contacts, and service plan meetings, etc.,
· Obtain collateral documents quarterly and/or annually as provided, from medical, mental health treatment, substance use and academic programs, and other programs.
· Responsible for preparation for family team conferences held at SSCF and/or in the community.
· Responsible for ensuring the notification and coordination with ACS, FSU workers, Mental Health, Health, and Substance Use providers.

· Ability to perform the essential functions of the job with or without a reasonable accommodation.
· Collaboration and facilitation skills that support the individual, families, and service provider’s needs.
· Demonstrates cultural sensitivity and competence in working with families from various ethnic and cultural groups, social economic status, and lifestyles.
· Ability to continue to learn modern technology and systems as needed.
· Knowledge skills and ability to adhere to acceptable ethical and behavioral standards of conduct.
Qualifications:
· Bachelor’s degree required.
· Three years of social service experience working within child welfare, substance use, mental health and/or domestic violence services preferred.
· Due to the nature of the work environment work hours will be based on needs of program and may vary.
· Performs other duties/responsibilities as assigned within scope of the work
