Key Responsibilities:
- Assess and identify the physical, psychosocial, and environmental needs of hospice patients.
- Develop and implement personalized care plans.
- Conduct initial and follow-up evaluations to ensure the highest standards of care.
- Communicate significant findings and changes in patient health to the care team.
- Educate patients, families, and care staff on hospice philosophy and pain management.
- Provide support at the time of death and perform bereavement assessments.
- Maintain patient confidentiality and adhere to HIPAA regulations.
What is Required?

- Reliable transportation to perform job duties
- Advantage Program Certificate, where state applicable
- Bachelor’s degree required, with additional one year paid professional experience, or one year as a Registered Nurse or Licensed Practical Nurse in lieu of an Bachelor’s degree.
- Preferred to Home Health or Hospice experience
- 50% travel
