Job Description:
Complete DX assessments
Completes and recommends monthly therapy and consultant hours
Prescribes medication accordingly and collaborating with pharmacies
Observes, records and reports PWS condition, reactions to prescribing medications, including PRNs.
Completes trainings and provide education to staff on medications, symptoms, side effects etc.
Reviews previous and current documentation, including discharge paperwork
Review EMAR
Document in Digital Agency and Behavior Agency
Collaborates with stakeholders including CCL, DDS and Regional Centers
Collaborates with medical personnel such as RN's, MD's, Social Workers, Psychologist and Psychiatrist in hospital or inpatient settings
Ensure best practices and provide discussions for the PWS with YAI RN's, Social Workers and QBMPs.
Attends House Meetings, Leadership Meetings, Clinical Rounds, and IBSPs.
Triage emergency and/or non-emergency phone calls
