Nurses needed to visit patients: Private Duty

Responsibilities:
- Provide nursing care to elderly, Children and/or disabled patients
- Assess patient needs and develop corresponding treatment plans
- Monitor patient status and notify office staff and supervising physician of significant changes
- Communicate with supervising physicians and other members of the care team to provide effective care
- Educate patients, family members, and caregivers on their conditions, diagnoses, medications, and treatments
- Ensure patient records are accurate and complete
- Serve as a patient advocate and provide advice and support to the patient and his/her family
- Record a patient’s medical history accurately
- Monitor patients’ condition including fluid intake and output and compose patient charts
- Provide emotional and psychological support when needed
- Communicate with patients’ family or friends to provide advice, comfort and release instructions
Qualifications:
- Previous nursing experience or Entry-level
- Strong communication skills and ability to teach
- Must be trustworthy, friendly, and dependable
- Strong understanding of nursing, medical skills, and care plans
- Must be self-motivated and able to work well independently
- MUST HAVE A VALID ILLINOIS LICENSE FOR LPN
Skills
- Proven experience as LPN or similar role or Entry-level
- Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
- Understanding of confidentiality obligations and nursing best practices
- Excellent knowledge of medical and hospital terminology
- Good knowledge of MS Office and data entry
- Perfect physical condition and stamina
- Excellent communication and interpersonal skills
- Compassionate and able to handle stress
- Successful completion of LPN licenses program is a must
