The Sepsis Coordinator plays a crucial role in collaboration with a multidisciplinary team to ensure effective management and coordination of care for patients diagnosed with sepsis. The Sepsis Coordinator performs various tasks to improve sepsis identification, treatment, and outcomes, focusing on patients’ optimal health, patient safety, and disease prevention. The coordinator provides sepsis education to patients, patients’ families, and hospital staff.
Sepsis Coordinator – The coordinator is a registered nurse who collaborates with the medical and hospital staff to provide multidisciplinary care to sepsis patients. Possess in-depth knowledge of sepsis, its pathophysiology, evidence-based interventions, and Sepsis Bundle Compliance Guidelines defined by the Centers for Medicare & Medicaid. Provide clinical and professional expertise for patients, families, and staff. The coordinator will collect and analyze data surrounding sepsis patients and lead the Sepsis Program including assistance in program development for treating all sepsis patients, research, and drive the implementation of evidence-based practice related to sepsis care. Responsible for reviewing concurrent medical records to facilitate the accurate documentation and treatment of sepsis care. This involves extensive record review, and interaction with clinicians, patients, and families to guide and educate sepsis care.
Quality – Collaborate with the Quality team to abstract, present, and promulgate quality measures.
Required Education:
Graduate of an accredited BSN Nursing program, clinical skills, mental development, and relevant experience that enables performance of all aspects of the position.
Required Experience:
- Minimum of three (3) years of acute clinical Intensive Care or Emergency Department experience.
- Working knowledge of Microsoft office programs.
- Experience with caring for the sepsis patient population, sepsis critical thinking skills.
- Strong organizational and prioritization skills.
Duties & Responsibilities:
- Monitor and evaluate sepsis cases within the healthcare facility, tracking patient outcomes and identifying areas for improvement to ensure that all sepsis patients receive consistent, comprehensive, and prompt treatment interventions.
- Facilitates and implement standardized evidence-based sepsis protocol practice across all disciplines in accordance with current sepsis guidelines to ensure compliance and consistency in diagnosis and treatment.
- Acts as a resource for staff and providers in developing a plan of care and support goals.
- Maintains current knowledge and competency in the management of the septic patient and in professional practice (e.g. conferences/workshops, professional organizations, etc.)
- Coordinates the multidisciplinary Sepsis program internally and within the community.
- Effectively work with others using an integrated approach toward achieving optimal patient outcomes in collaboration with nursing educators to conduct ongoing training sessions for healthcare professionals, including nurses, physicians, and support staff, to enhance their understanding of sepsis identification, early intervention, time-zero determination, documentation best practices, and evidence-based treatment.
- Collaborate with various departments, including physicians/providers, nursing, emergency departments, intensive care units, pharmacy, and laboratory services, to streamline communication and optimize sepsis care pathways.
- Role models critical thinking by demonstrating active and proactive problem solving.
- Provide real-time sepsis compliance rates and feedback to all clinicians.
- Leads performance improvement processes recognizing trends, exploring alternatives and adapt to changes by incorporating new guidelines and front-line feedback.
- Develops and implements sepsis related policies and procedures reflecting current evidence
- Possess goals to decrease sepsis mortality, LOS, and readmission rates.
- Interprets and presents data for key stakeholders - Collect and analyze data related to sepsis cases, including patient demographics, time to treatment, and clinical outcomes, to identify trends, mortality, LOS, readmission rates, and opportunities for improvement.
- Develop innovative initiatives to address barriers and gaps. Utilizes critical thinking to implement quality improvement initiatives based on data analysis findings, and feedback to enhance sepsis care processes and outcomes.
- Identifies learning needs of professional staff and patient education
- Provides education in order to advance staff, patient, and their families’ knowledge
- Collaborates in the development and implementation of the interdisciplinary education plan
- Maintains staff competency with regulatory requirements
- Community Engagement - Develop a sepsis community program focusing on the sepsis continuum of care and prevention while bringing awareness to sepsis implications.
- Regulatory Compliance - Acts as the hospital’s subject matter expert: Stays updated on sepsis-related guidelines, regulations, and best practices, ensuring compliance with relevant standards and accreditation requirements. Continuously seek to improve sepsis bundle compliance rates and submit the rates to CMS. Collaborates and shares new information, guidelines, and deadlines to all stakeholders.
