About the Role: We are seeking a dedicated and compassionate Nurse Practitioner to join our team dedicated to providing comprehensive healthcare services. The successful candidate will play a crucial role in diagnosing and managing a wide range of health issues, providing personalized care, and promoting overall wellness among our patients. Core Responsibilities:
Conduct comprehensive patient assessments, develop treatment plans, and manage patient care to improve clinical outcomes.
Perform physical examinations, order and interpret diagnostic tests, and provide clear explanations of medical conditions and treatment options.
Prescribe medications and other therapies, monitor efficacy, and make adjustments as needed.
Collaborate with physicians and other healthcare professionals to coordinate patient care.
Educate patients and their families on disease prevention, health maintenance, and medication management.
Maintain timely and accurate patient records through our electronic health record system.
Participate in continuous quality improvement initiatives and stay updated with advancements in medical practices and technologies.
Perform physical examinations of patients to evaluate health and diagnose illnesses
Order and perform diagnostic tests, such as x-rays and laboratory tests
Determine, plan, and initiate appropriate treatment, and adjust treatment if necessary
Prescribe and dispense medications
Document medical information of patients and maintain electronic medical records, currently using Tebra EMR.
Educate, instruct, and counsel patients and families on presenting health problems, treatment plans, and home care, and encourage and promote preventative health care and positive health behaviors
Request consultation or referral with other health care providers, such as physicians and clinics
Actively maintain knowledge of current medical research and trends, and support research projects

Qualifications & Requirements:
- Minimum 3 years experience as a Certified Nurse Practitioner.
- Experience in Primary Care, Internal Medicine or Geriatrics (preferred).
- A desire to provide excellent Patient Care.
