Cascade Park Communities is seeking a Licensed Practical Nurse (LPN) to join our team at Cascade Park Gardens, an assisted living facility specializing in our Behavioral Health Horizon program. We are dedicated to providing compassionate care for elderly and mentally ill individuals. The ideal candidate will be passionate about enhancing our residents' lives through quality care and will play a crucial role in our community.
Responsibilities:
- Monitor the health and well-being of residents, with a focus on dual diagnosed Mental Health and geropsychiatry residents.
- Perform Diabetic Management, including blood sugar checks and insulin administration.
- Coordinate and communicate effectively with external healthcare providers such as primary care physicians and home health agencies.
- Maintain open communication with family members, Power of Attorneys, and legal guardians.
- Provide clinical input for care conferences and participate in care planning.
- Supervise and oversee the care provided by caregivers and medication technicians.

Qualifications and Skills:*
- Current LPN license.
- Experience in nursing care practices, particularly in geriatric care and knowledge of state Boarding Home regulations.
- Training in Therapeutic Options & Hand in Hand is advantageous.
- Excellent interpersonal skills with the ability to work in a dynamic environment.
- Reliable, team-oriented, compassionate, and committed to fostering an environment of dignity and respect.
- Positive attitude, high energy level, and strong communication skills.
- Detail-oriented with a dedication to delivering high-quality care.
