POSITION DESCRIPTION
TITLE: Medical Social Worker
DEPARTMENT NAME: ALWP
REPORTS TO: Director of Case Manger and Executive Assistant to Concise Care Group
*You must be a local resident. No out of state applicants.
We're seeking a person to assist with monthly assessment at assisted living e with the seniors population. Familiar with Medi-Cal housing programs. experience in an Assisted Living Facility working with the elderly. You have networked and developed relationships. You're well-versed with the Assisted Living Waiver, Regional Centers and PACE programs.
A. DUTIES AND RESPONSIBLITIES
1. Overall Care Management of participants of the Assisted Living waiver (ALW) & Regional center programs.
2. Familiarity with the Assisted Living Waiver (ALW), Regional Center and PACE Programs.
3. Assist residents with Medicare & Medi-Cal benefits. Familiar with Social Security and Medical resources.
4. Maintains effective communication with residents, family, Service coordinator, Social worker, Clinical Supervisor, and other health care team members in regard to residents' needs and progress.
5. Develops the Appraisal of Needs/Service plan for program participants.
6. Ensures enrollment status remains current for ALW and Sales clients
7. Ensures that brochure requests are mailed and followed up on.
8. Ensure that all communications is documented in CRM platform; Casebook.
9. Obtain a weekly Update for Case Manager Availability, and update Programs about vacancies.
10. Submit a Weekly Report of Projected and Actual Plan.
11. Build community awareness by contacting community resources, centers, and groups.
Assist residents with connecting with resources.
12. Coordinates with the Activities Dept. to have activities that advance community outreach.
13. Maintains liaison with families and residents.
14. Complete Assessments (functional capability, head-to-toe) for prospective and current residents.
15. Weekly ALW social worker meetings.
16. Maintain the following reports: a. Track program participants that are in SNF's/hospitals. Follow residents admitted to
SNF/Hospital & keep up-to-date on progress/dc plans b. Track wellness program c. Track Medi-Cal Reinstatement. d. Track census e. Track ALW Assessment Deadlines & Tier Levels f. Makes rounds each morning to ensure that the facilities are ready for tours
B. MINIMUM QUALIFICATIONS
1. Develop, close relationships by networking with hospitals and discharge planners for prospective new residents.
2. experience working in an assisted living facility environment.
3. Presents a professional image to prospect through dress, behavior and speech.
4. Experience working with older adults and seniors.
5. Possesses Common sense.
6. Familiarity with the Assisted Living Waiver Program (ALW) and Regional Center Programs
7. experience in a similar role in the healthcare industry, capacity for seniors/LTC settings.
8. Ability to work well with our demographic: seniors and adults with developmental, cognitive, or memory impairments.
9. Current training in First Aid and CPR.
10. Good written/verbal communication skills
11. Valid driver's license
12. Familiar with Social Security and Medi-Cal resources.
13. Professional appearance and positive attitude
C. SPECIAL SKILLS NEEDED TO PERFORM THE JOB
1. experience within an Assisted Living Facility environment. Highly energetic and organized individual who has a positive, "take ownership" attitude.
2.Highly competent to work effectively with families/prospects seeking non-medical care.
3.Have sufficient background knowledge and expertise in assessment, evaluation and skills to meet the needs of his/her residents and to contribute to Quality Management review and evaluation

1. To administer the admissions function in accordance with Title 17, Title 22 and company policy and procedures.
