DESCRIPTION OF DUTIES/ESSENTIAL FUNCTIONS
The Counselor position at the Houston Health Department – Community Reentry Network Program will:
Provide oversight for behavior health and other basic needs services for the CRNP as needed
Facilitate case staffing for program participants
Interview, screen and assess applicants' eligibility for program participation.
Create case plans, evaluate needs and concerns and make referrals to appropriate community service/counseling organizations.
Maintain and expand relations with the social services community to ensure clients' needs are met.
Maintain accurate and timely client records ensuring data is included for effective follow-up and required reports.
Facilitate assigned curriculum and groups as needed
Design, prepare and deliver presentations to the community to address program interest and support.
Design, prepare and present counseling services training and orientation to newly enrolled participants and families
Provide intervention for participants experiencing more sensitive and/or complex problems.
Ensure appropriate community referrals are made and documented; Conduct scheduled follow up to determine outcome of referrals
Conduct counseling and case management activities.
WORKING CONDITIONS
The position is physically comfortable; the individual has discretion about walking, standing, etc. There are no major sources of discomfort, i.e., essentially normal office environment with acceptable lighting, temperature and air conditions. Requires the ability to make simple gross motor responses within large tolerances.
EDUCATIONAL REQUIREMENTS
Requires a Bachelor's degree in a social or behavioral science, such as Sociology, Psychology, Social Work or Counseling.
EXPERIENCE REQUIREMENTS
Two (2) years of professional experience in counseling or related social services work are required.
A Master's degree in a social or behavioral science may be substituted for the experience requirement.

Working knowledge of physical health, behavioral health and social service systems.

Solid background in coordinating community service collaborations and other outreach projects.

Knowledgeable of community services, supports and eligibility criteria.

Excellent communication skills – verbal and written.

Ability to manage multiple projects and work with diverse partners.

Computer savvy with advanced skills in Microsoft Office products.

Five or more years of professional experience in providing counseling, case management and referrals for formerly incarcerated individuals
SELECTION/SKILLS TESTS REQUIRED
The selection process will involve application review and/or interview. The department may administer a skills assessment test.

GRANT FUNDED POSITION
Grants & Special funded positions are dependent upon continued available funds. If funding is no longer available, employee may be laid off or transferred. Factors used in determining the salary offered include the candidate's qualifications as well as the pay rates of other employees in this classification.
