OVERVIEW:
The Home Health Administrator position provides support for CCRC’s residents; Manages the IL Wellness Clinic; Provides follow-up to IL residents who have required support from facility emergency/urgent response service; and helps manage the Team Member Health Nurse Process for St. Mark Village employees.
RESPONSIBILITIES: Include the following. Other duties may be assigned.
· Fully responsible for the management and services including budgeting, planning, recruiting and fiscal management of our private duty home care agency, and the skilled home health agency.
o This position wears many hats but is mainly responsible for setting the St. Mark Village standard for culture, quality, teamwork, and success in this department.
o Monitor the quality and appropriateness of all services provided by your staff to ensure compliance and client satisfaction while ensuring adequate staff education, training, and evaluation.
o Has responsibility for seeing that all standards are met and that we are in compliance with all federal, state, accrediting bodies, and local agency policies.
o Assure that the department is staffed to care for the departmental needs.
o Help create, prepare and maintain current policies and procedures.
· Provide support, for IL Residents:
o Help assess IL Residents situations and guide them to the available internal and third-party services available to them that would best help them in each situation
o Welcome visit for all newly admitted IL Residents (orientation to services)
o Provide direction for planning surgery and follow-up
o Assist IL Residents when they transfer internally between levels of care and when returning to IL apartment to support continuity of care
o Provide information on third party services, etc.
· Manage the Wellness Clinic
o Monitor vital signs
o Monitor weight
o Review Physician directions
o Review medical documentation
· Provide follow-up to IL Residents who have required support from facility emergency/urgent response service:
o Visit in Independent Living apartment for follow-up
o Direct residents to make physician or related medical appointments when needed
o Provide information on third party services
o Communicate with the family and other interested persons at the direction and/or approval of the Independent Living Resident
· Manage Team Member Health Nurse process
o Assist with pre-hire and annual TB testing
o Vaccine clinic coordination
o Other possible team member health processes and procedures
· Assist IL Residents who are transitioning to another level of care when needed.
· Visit IL Residents in local hospitals when requested.
· Observes and follows safety policies and procedures.
· Observes and follows infection control policies and procedures.
· Attends all mandatory in-services as required.
· Demonstrates positive guest relations to internal and external customers.
· Report any incidents of actual or possible abuse of a Resident that they become aware of. Abuse in this respect includes, but is not limited to, verbal, sexual, physical, and/or mental abuse, corporal punishment, involuntary seclusion, neglect, and/or any misappropriation of property concerning St. Mark Village Residents. Team Members will make the report as soon as possible after they become aware of the incident of actual or possible abuse.
· May be assigned other related duties and responsibilities as required by the VP of Healthcare Services,
SUPERVISORY RESPONSIBILITIES:
Carry out supervisory responsibilities in the Assisted Living Center, as needed, in accordance with the facility’s policies and applicable laws. Responsibilities may include; as needed planning, assigning, and directing work; providing input into appraising performance; may be involved in providing input into Team Member discipline; addressing and resolving complaints in coordination with the Department Director.
REQUIRED SKILLS, EDUCATION AND/OR EXPERIENCE:
· Florida Registered Nursing License (R.N.); minimum three to five years related experience and/or training; or equivalent combination of education and experience.
· Three years of previous Home Health Management experience preferred.
· Must demonstrate excellent communication skills both verbally and electronically. Must Possess thorough organizational skills, as well as an ability to work well with a Team.
· Must have sound clinical judgment.
