Job Summary:
Physical Therapy Assistants work under a physical therapist to help our residents with physical injuries or illness of to regain range movement and control pain. Under the PT’s direction and plan of care, the PTA helps residents who have lost motor function due to accidents, illness or age. FULL-TIME NEW SIGN-ON/RENTENTION BONUS PROGRAM

Responsibilities and Duties:
- Works under the direction of the Physical Therapist to plan, implement, or administer programs that restore or enhance performance in individuals with functional impairments.
- Assists Physical Therapist with implementing treatment plans.
- Reports to supervisors on patients' progress and discharge recommendations.
- Attends and reports accurately a patient’s status at weekly UR meeting and interdisciplinary care plan meeting.
- Observes and records patients’ progress, response to treatment and limitations and maintains this information in electronic medical record.
- Completes required documentation at point of service.
- Identifies need for modification of treatment plan and reports to the Physical Therapist.
- Educates staff, family and caregivers as indicated.
- Identifies needs for program development and implements programs according to company policies and procedures.
- Coordinates in-services for facility staff as need is determined, to ensure safe and effective carry over of Restorative Programs and overall call of the residents.
- Takes a patient centered approach to provision of care.
- Carry out all job responsibilities in a professional manner at all times, upholding excellence in quality of care.
Qualifications:
- Education: Graduate of an accredited therapy program
- Licenses/Certifications: Current Physical Therapy Assistant state license
- Maintain state and national licensure requirements
- CPR certified
- Service orientation
- Active learning
