The Hospice RN plans, organizes, and directs hospice care provided by the Interdisciplinary Team, with emphasis on community health, education, and experience. The Hospice RN uses available resources to plan and direct services to meet the needs of patients and families within their homes. EEO.
You will be working with other members of our Hospice Care services team to provide the very best holistic end-of-life medical, nursing, spiritual, and emotional support to patients with life limiting illnesses, when a cure is no longer possible.
The Hospice Registered Nurse delivers quality hospice and palliative nursing care to patients in the community, nursing home, or ALF setting as needed outside of traditional business hours.
*Essential Functions:*
* Collaborate with physicians and other professionals to plan care.
* Make frequent visits to the patient to evaluate their condition.
* Provide support and comfort according to individual patient’s needs.
* Work with caregivers to plan and execute excellent patient care.
* Alleviate sudden symptoms of the disease’s end phase.
* Notify a doctor of a patient’s condition when deemed necessary.
* Assist patients and their families with paperwork.
* Maintain accurate records.
* Help relatives cope with the upcoming loss and provide post-mortem support
*Minimum Position Requirements*_:_
* Demonstrates a passion for the mission of Catholic Community Hospice
* Graduate of an accredited school of nursing. RN licensure (current, unrestricted).
* Excellent communication and rapport building skills, and computer proficiency are essential.
* Must possess a valid Kansas or Missouri Driver’s license, dependable transportation, current vehicle insurance, and a verified safe driving record.
