POSITION SUMMARY
The Client Navigator is responsible for the connecting of quality patient care. Works to remove barriers to care by direct contact and collaboration with management and clinical team. Follows-up with clients in person and via telephone to ensure adequate use of services and connections with internal resources. Collects relevant data and generates reports for management case assignments. This position reports to the Clinical Site Manger

- Accurately maintains accurate and up-to-date patient records and program files; maintains databases for grant-funded activity reporting

- Uphold and consistently represent the values, mission, and policies of the organization at all times.
PRIMARY TASKS AND DUTIES
· Assists management in identifying underlying barriers to access for new patients.
· Works with program management to improve client workflow
· Accurately evaluates the needs of referred clients from different sources, facing different obstacles.
· Suggests new ways to meet the needs of patients; pursues these options with creativity.
· Monitors timeliness of response to various types of clients; communicates effectively with team members and other staff involved in program to remove barriers and insure timely response.
· Brings clients, parents, etc. into active contact with clinical staff, etc.
· Verifies that clients completed care – attended appointments, follow-up, etc.; conducts necessary follow-up with clients.
· Understands and markets various services and programs to clients, based on case-by-case client interviews and individual circumstances.
· Other duties as assigned.
ESSENTIAL FUNCTIONS/KEY COMPETENCIES
· Demonstrated ability to work in Athena and use other office equipment effectively.
· Able to communicate with staff and patients effectively.
· Knowledgeable of outside resources the patients can use
Education
· High School diploma or equivalent.
· Experience
· 1 to 2 years experience
· Administrative Experience.
· Must be able to speak, read, write and understand English.
