ESSENTIAL JOB FUNCTIONS AND DUTIES includes but are not limited to the following:
Provides and is responsible for provision of social services in accordance with the physician’s plan of treatment, acceptable standards of practice and established policies and procedures.
Assists the physician and other members of the treatment team to understand the significance of social and emotional factors related to the patient’s care and participates in development of treatment plan
Intake or admission screening
Psychosocial assessment of new patients; developing an update or detailed reassessment of the patient
High-social risk case finding
Maintains contact with family and significant others in the patient’s life when appropriate and provides patient and family education, support, and advocacy
Provides coordination and acts as liaison with community based social and mental health agencies
Reassesses social and emotional factors in order to help the patient and family cope with problems in daily living to help them follow medical recommendations to maximize the patient’s health condition
Counsels patients and families concerning the facilitation of the medical plan of treatment
Recognizes and utilizes community and family resources to assist the patient in long-term plans for life in the community or to learn to live within the patient’s disability.
In conjunction with the physician, patient, and patient’s family, develops an appropriate plan of care dealing with personal, financial, and environmental difficulties that may interfere with the health and wellness continuum.
Facilitates discharge planning and conducts post discharge follow-up to assure that linkage of the patient with community resources has occurred reducing re-hospitalization
Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness, and according to policies. Submits accurate documentation within mandated time frames.
Oversees the quality and appropriateness of service provided
Develops mechanisms for exchange of appropriate information with sources outside of the hospital
Keeps abreast of social work trends and knowledge and participates in in-service programs
Coordinates the total plan of care and maintains continuity of patient care conferences for complex and/or multidisciplinary clients when needed or helpful to ensure good coordination of care.
Participates in performance improvement program.
Cooperates with appropriate staff to provide staff education when requested and arranged by the Director of Nursing
Stays abreast of federal, state, and local laws and applicable regulations including confidentiality guidelines
Advises Director of Nursing/Administrator of any risk management activities in order to reduce or eliminate patient/staff/visitor injuries, illnesses, death, and/or financial loss.
MINIMUM EXPERIENCE REQUIREMENTS :
Two years experience of social services provided in an inpatient/outpatient setting including but not limited to discharge planning, family and/or group psychotherapy with adults, and treatment team participation.
MINIMUM EXPERIENCE REQUIREMENTS :
Level of knowledge acquired through the completion of a masters level degree in social work (MSW) from an accredited school of social work. Unrestricted LCSW licensure required.
