SUMMARY OF JOB:
The Nurse Liaison is responsible for following up on the Sisters of Saint Joseph who are in the hospital and communicating their needs/condition to the Villa, Congregational President and the biological family. In addition, the Nurse Liaison will support the Villa nursing team with communication to the Congregational President, and biological families.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Follow up on all Sisters that are in the hospital including discussion with hospital staff and physicians.

Attend doctor appointments as requested for those Sisters who need to be escorted by a nurse.

Communicate the information received from medical staff with the Villa and the Congregational President to make appropriate medical decisions.

Communicate the medical updates to the Sister’s biological family.

Communicate with and coordinate the companion Partner in Mission.

Attend care conferences as needed/upon request.

Notify Pastoral Care about the Sister’s condition as needed.

Maintain a working relationship with Congregational Liaison and Sister Advocate.

Documentation in electronic health record as needed.

Understanding and commitment to the Sisters Saint Joseph Vision, Mission and Values.

Knowledge of Ethical and Religious Directives for the Catholic health services.

Knowledge and desire to care for, support and serve the Sisters with compassion, sensitivity and respect for their care management needs.

Support the nursing team with communication of care updates to the biological families.

Able to work with Partners in Mission and Companion Partners in Mission.

Good communication, organizational and leadership skills.

Ability to make sound medical decisions.
