Job Summary
To assure that Clients who are functionally limited in performing ADLs receive the highest quality
in - home technical care possible by ensuring that the necessary services are delivered by qualified health care providers thus promoting Client satisfaction , safety and well-being.
Responsibilities and Duties
- Reports to the Director of Rehab Services.
- Supporting the Agency's mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.
- Demonstrating clinical competency in physical/occupational/speech therapy skills.
- Visiting patients in a timely manner, in accordance with Home health department guidelines.
- Developing an appropriate plan of care based on interpretation of skilled evaluations, physician's orders and the patient's individual needs.
- Providing evidence-based therapy interventions and assisting in monitoring patient progress through observations and examination
- Evaluating the effectiveness of therapy in accomplishing goals and intended patient care outcomes
- Communicating significant changes in patient status to the physician, rehab director and the appropriate Home Health Department personnel when indicated.
- Demonstrating effective communication skills in patient education, instructs patient/caregiver in aspects of home safety.
Qualifications and Skills
Doctor of Physical Therapy, Bachelor or Master in Physical Therapy. Current active Physical Therapist license in Texas. Home Health experience preferred, new graduate welcome to apply. Bilingual is a plus.
