Peace Aging Care Experts was established 18 years ago by Trish Colucci Barbosa. We provide premier private pay care management services to seniors, and adults with mental illness, and occasionally adults with disabilities. Our mission is to help families identify and connect with the resources they need to ensure the safety and well-being of their elderly or vulnerable loved ones. Our goal is to bring reassurance & peace of mind to all of our clients, their families, and the professionals we interact with. Our team is growing, supportive, and flexible.
We are currently looking to add additional Care Managers in Warren, Sussex and Morris Counties. We look forward to hearing from you and adding another Care Manager to our team!
About Per Diem:
At Peace Aging Care Experts, our care managers are W2 employees who work “per diem,” meaning they are paid for the billable time they generate while working with clients (this includes making/answering phone calls, emails, research, driving to clients, visiting with clients, attending doctor appointments, etc.). Care Managers typically work 15-30 hours per week depending on their preferences, their current client assignments, and clients’ needs (which do fluctuate). Because our clients count on our responsiveness and ability to handle situations quickly, it is best if this job is the only position the candidate holds.
Work Location:
Our Care Managers operate out of their home offices. A company laptop and cell phone are provided for work use. Peace Aging Care Experts’ full service area covers the following counties: Morris, Bergen, Hudson, Essex, Union, Middlesex, Passaic & Somerset. Clients are assigned to care managers based on the clients’ needs and geographical location to ensure that Care Managers are not driving unreasonable distances for visits.
Job Responsibilities:
Meet with our new clients to conduct initial assessments (determine medical, social, emotional needs)
- Create a Plan of Care for our clients’ current and future needs
- Build a relationship with our clients to become a trusted member of their care team
- Manage our clients’ care ongoing (with a focus on their medical, emotional and social needs)
- Communicate regularly with our clients’ families and/or responsible parties
- Schedule & attend our clients’ medical appointments
- Manage client medications & ensure refills are available
Qualifications:
- Required: Registered Nurse (RN), Licensed Social Worker (LSW), Licensed Physical Therapist, or Licensed Occupational Therapist
- Required: Caring individual who enjoys helping clients and their families
- Preferred, Not Required: Certified Case Manager (CCM) or Care Manager Certified (CMC)
