Your Responsibilities:
- Holistic Patient Assessment: Assess patients' physical, psychosocial, and environmental needs to develop comprehensive care plans tailored to their individual requirements.
- Hands-On Care: Provide direct patient care, ensuring the delivery of high-quality services aligned with Elara Caring's mission and standards.
- Effective Communication: Collaborate with interdisciplinary teams, physicians, and facilities to communicate patient status, changes, and care plans effectively.
- Patient Education: Empower patients and their families with knowledge on hospice philosophy, pain management, and symptom control.
- Compassionate Support: Offer emotional support and counseling to patients, families, and caregivers throughout their healthcare journey.
- Continuous Improvement: Regularly review and revise care plans based on patient needs, ensuring optimal outcomes and quality of care.
Qualifications:
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100lbs
- Driver's License
- CPR/BLS
- Strong communication skills
- Reliable transportation to perform job duties
- Bachelor’s degree required, with additional one year paid professional experience, or one year as a Registered Nurse
