Job Title: Certified Dietary Manager (CDM)
Job Status: Non-Exempt
Job Type: Part-Time (24 hours/week, occasionally 32 hours/week)
We are hiring a Certified Dietary Manager for a part-time role in a long-term care facility. The Certified Dietary Manager is responsible for the maintenance and updating of records concerning the diets, nutritional care plans, assisting in maintaining quality nutritional levels for residents, and other dietary needs of residents.
Duties and responsibilities include, but are not limited to:
1. Demonstrates dependable regular and consistent in-person attendance and punctuality.
2. Consults with the Registered Dietitian, Director Dietary Services, and nursing staff in maintaining and assessing the nutritional care programs for residents.
3. Performs routine nutritional assessments, as needed and/or scheduled.
4. Calculates nutritional intake (e.g., calories and protein) for calorie counts and provides such information to Registered Dietitian for assessment.
5. Gathers information from wound round data, chart reviews, and nursing reports.
6. Performs weight checks as indicated.
7. Reviews weight reports in Point-Click-Care on a daily basis for weight loss issues and notifies the Registered Dietitian of any adverse findings.
8. Ensures progress notes that are charted are accurate, descriptive, and informative of the resident services provided.
9. Obtains the dietary food preferences of each resident admitted to the facility and assists the Registered Dietitian in planning for the resident’s prescribed diet plan. Interviews family members as necessary to obtain diet preferences and history. Maintains records of residents’ dietary likes and dislikes.
10. Enters resident care plans, revisions to plans, and MDS into the Point-Click-Care system. Processes and charts diet and supplement changes along with new diets.
11. Participates in discharge planning, development, and implementation of resident care plans, resident assessments, etc., as required.
12. Attends quarterly QAPI meetings, or more frequently is needed. Participates in QAPI audits and data collection. Serves on other committees as required.
13. Performs clerical duties such as completing necessary forms and writing reports, or other related duties as necessary or as directed.
14. Immediately reports hazardous or unsafe conditions.
15. Acts a resource for questions regarding resident diets and nutrition.
16. Cooperates and uses teamwork with coworkers to achieve objectives. Collaborates as a team with personnel from other departments in relation to resident care. Treats others with respect and shows sensitivity for cultural differences. Displays a patient, approachable, and friendly disposition.
17. Complies with HIPAA regulations and procedures. Ensures and maintains privacy, confidentiality, and security of protected health information.
18. Uses reason and observation to identify and resolve problems in a timely manner. Gathers, analyzes, and synthesizes complex information and variables to generate solutions and alternatives. Works collaboratively in group problem solving situations and builds a positive team spirit. Focuses on solving conflict, not casting blame. Keeps emotions under control.
19. Immediately reports actual or suspected of instances resident abuse, regulatory violations, and crimes using the reporting methods outlined in the facility’s compliance and ethics program.
20. Immediately reports accidents or near-misses to self or others, no matter how minor, to a Supervisor or the department Manager. Reports all safety violations and hazardous conditions or equipment. Follows all safety policies and protocols.
21. Complies with the facility’s policies and procedures, including, but not limited to, the employee handbook, the compliance and ethics program, safety policies, HIPAA, and other facility policies and State and Federal laws. Supports the departments and the organization's goals and values.
22. Attends and participates in continuing educational an in-service programs. Pursues training and development opportunities. Strives to continuously build knowledge and skills.
23. Uses appropriate PPE (e.g., masks, gloves, goggles, gowns), as required.
Required Qualifications
- High school diploma or equivalent.
- CDM certification.
- One year minimum experience in a food service operation. Clinical dietary experience in a hospital or nursing home a plus.
- General PC skills. Proficiency with MS Word and MS Excel preferred. Proficiency with Point-Click-Care or other EHR a plus.
- Ability to make independent decisions and exercise good judgment.
- Ability to deal tactfully and respectfully with residents, family members, visitors, and coworkers with patience, tact, and a cheerful disposition.
- Knowledgeable of best practices and procedures, as well as related laws, regulations, and guidelines governing dietary functions in long-term care.
- Ability to plan, organize, and implement a variety of dietary programs, goals, objectives, policies, and procedures.
- Ability to read and interpret dietary information and resident care plans, while effectively communicating a resident's condition, whether verbally or in writing
- Ability to function independently, and with integrity, in times of minimal supervision.
