We provide comprehensive, integrative, and evidence-based services that address the physical and emotional needs of our residents.
Qualifications and Competencies
A minimum of two years of work experience as a Housing Case Manager is required and experience using the HMIS system and similar data systems such as CaseWorthy, FYI, HUD Exchange, etc.
Must have experience in Public Housing Sector; Permanent Supportive Housing (i.e. Emergency Housing, Rapid Re-Housing, and ReEntry Housing Program)
Position Overview and Job Duties
Housing Case Manager
The role of a Housing Case Manager is a professional who works with individuals or families to help them secure and maintain stable housing. The primary role is to assess the housing needs of their clients, connect them with appropriate housing resources, and provide ongoing support to ensure housing stability:
- Provide direct support and assistance to individuals or families experiencing housing instability or homelessness.
- Conduct comprehensive assessments to determine housing needs and barriers.
- Develop and implement individualized housing plans, considering factors such as affordability, accessibility, and support services.
- Monitor and evaluate the progress of clients in securing and maintaining stable housing.
- Advocate for clients and connect them with relevant resources, including housing assistance programs, financial counseling, and social services.
Assessment:
- Conduct thorough assessments of clients' housing situations, including their current living conditions, financial situation, and any barriers to housing stability.
Resource Referral:
- Identify and connect clients with available housing resources, such as affordable housing programs, rental assistance programs, and supportive housing services.
Advocacy:
- Advocate on behalf of clients to navigate housing systems, resolve housing-related issues, and address any legal or bureaucratic barriers they may face.
Case Planning:
- Develop individualized housing plans with clients, outlining specific steps and goals to achieve stable housing. This may include addressing issues such as budgeting, employment, and mental health.
Coordination:
- Collaborate with other service providers, community organizations, and government agencies to coordinate comprehensive support services for clients, including healthcare, employment assistance, and social services.
Education:
- Provide clients with information and education on topics such as tenant rights, budgeting, and housing responsibilities to empower them in maintaining long-term housing stability.
Crisis Intervention:
- Assist clients during housing crises, such as eviction threats or homelessness, by providing immediate support and helping them access emergency housing resources.
Documentation:
- Maintain accurate and up-to-date case records, documenting client progress, services provided, and any relevant information to ensure accountability and continuity of care.
Monitoring:
- Regularly monitor and evaluate clients' progress in achieving housing stability, adjusting plans and interventions as necessary.
Empowerment:
- Empower clients by fostering self-sufficiency and independence, helping them build the skills and resources needed to maintain housing stability in the long term.
The Housing Case Manager role demands a blend of leadership, housing expertise, case management skills, and a deep understanding of the complex challenges individuals or families face in securing and maintaining stable housing.
