Hospice Registered Nurse
Location: Sacramento, CA
Shift: Full-Time
Salary: $94,000- $125,000
Summary:
Seeking a compassionate and experienced Registered Nurse to join our client's hospice team. You'll play a key role in planning, directing, and providing comprehensive care for patients and families at home and within the community, with a focus on palliative nursing and community health education.
Responsibilities:
- Patient Care: Conduct initial and ongoing assessments, develop and implement individualized care plans, manage pain and symptoms, provide hands-on nursing care, and support daily living activities.
- Communication: Collaborate with physicians, families, and community partners, document patient progress, and participate in interdisciplinary team meetings.
- Education: Equip patients and families with self-care skills, provide medication and diet instructions, and offer emotional support and counseling.
- Teamwork: Foster a safe and supportive environment, delegate tasks to ancillary personnel, and actively participate in quality improvement initiatives.
- Professional Development: Maintain current licensure, attend continuing education, and continuously refine your nursing skills.
Qualifications:
- RN license with current state licensure.
- Minimum 2 years of experience in public health, home care, or hospice nursing (preferred).
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and prioritize effectively.
- Strong organizational and time management skills.
- Valid CPR certification.
- Must have reliable transportation and driver's license.
