- Contract Length: 13 weeks
- Start Date: ASAP (bi-weekly orientation)
- Guaranteed Hours: biweekly guarantee- 72/80 hours – 1 cancellation allowed per 2-week period
- Shift Requirements: 8 HR Days (8am-4:30pm)
- Float Requirements: N/A
- On Call/Call Back Requirements: Saturday coverage in house for 4 hours (available by on call for another 4 hours) making an 8-hour total coverage. ONCE every third/fourth weekend.
- Weekend/Holiday Requirements: Will follow hospital schedule- can obtain from HR, typically every other rotation.
- Required Certifications to be Attached upon Submittal: must have a MSW/LSW (IL).
- Minimum Experience: at least 1+ year in hospital. Medical background/DC planning (ED, ICU, Tele, M/S). Must have experience working in a medical hospital setting with an interdisciplinary team & discharge planning.
- Charting Experience Required: we are currently using Expanse, training will be provided. Must be able proficient in Microsoft Word
- Required Skillset: hospital social work/case management (will train further upon onboarding)
- Unit Information (Bed size, Case type, Patient ratio): Individual must be able to handle caseload of 20-25 medical patients effectively.
