Position Overview: Remote Mental Health Case Manager
As an integral part of our team, you will hold a key position responsible for delivering professional case management services. Our organization is committed to making a lasting impact on individuals' lives, empowering them to overcome mental health challenges and lead fulfilling lives.
Location: Fairfax, VA
Residency Requirement: Must be permanent residents within the Greater Houston area.
Key Responsibilities:
Conduct thorough assessments and management services, utilizing validated rating scales. Craft individualized behavioral healthcare plans that address behavioral/psychiatric concerns. Adapt plans for patients showing insufficient progress or experiencing changes in their condition. Provide concise psychosocial interventions.
Collaborate closely with the care team and maintain accurate records in the registry. All activities are conducted in consultation with the psychiatric consultant.
While primarily a remote position, availability for in-person service provision to beneficiaries is required. Cultivate lasting relationships with beneficiaries and foster a collaborative, integrated rapport with the broader care team.
Demonstrate flexibility by engaging with beneficiaries beyond regular clinic hours when necessary to fulfill the role of a behavioral health care manager.
Serve as a patient advocate within the treatment team, offering indirect support and acting as a liaison on their behalf.
Qualifications and Requirements:
Bachelor's degree in Psychology, Social Work, Counseling, or a related field (Master's degree preferred).
A minimum of 2 years of experience in a behavioral health setting.
Relevant certifications or licensure in mental health (e.g., Certified Mental Health Counselor, Licensed Professional Counselor) are advantageous.
In-depth understanding of various mental health conditions, therapeutic interventions, and crisis management techniques.
