ESSENTIAL FUNCTIONS:
Under the supervision of the Program Manager, Mental Health (MH) Therapist conducts mental health assessments & treatment plans (via the IM+CANS), and provides individual, family, and couples psychotherapy – versed in Trauma, as well as other supportive services – to individuals suffering from a wide range of mental health disorders. This position requires willingness to work 2-evenings/week to accommodate clients requiring after work/afterschool appointments. Access to reliable transportation, or comfort & familiarity using public transportation, will be necessary for this position.
RESPONSIBILITIES:
- Complete an Integrated Assessment & Treatment Plan (IATP) using the State mandated IM+CANS tool with all new clients.
- Conduct primarily individual MH therapy sessions, with family and couples therapy sessions being provided, as deemed necessary & appropriate to the client’s treatment protocol.
- Maintain case files (i.e., clinical documentation) for all clients assigned to caseload, with all files being subject to regular internal quality assurance audits;
- Work as part of a team to further develop program initiatives and to increase program referral network.
- Perform community outreach & advocacy work with the goal of increasing the visibility & understanding of Re-Entry Barriers, Mental Health and Substance Use Disorders, and to promote community treatment options.
- Identify and provide referrals for additional services, as needed.
- Engage in regular clinical peer review of cases, particularly those involving the potential risk for self-harm or harm to others (particularly in cases where an infant or child could potentially be at risk).
- Attendance Group Supervision/Team Clinical Case Review meetings/Discharge Reviews/COD.
- Will provide support for re-entry navigation based on participants identified area of need using scales to determine Barriers to Reentry and SDoH.
- Participate in agency sponsored trainings, agency meetings, community outreach events, health fairs, fundraising events, etc., as directed by the Program Director.
- Perform all other duties as assigned by the Program Director.
QUALIFICATIONS:
- A Master’s degree in Social Work, Counseling, Psychology, or another related field, from a regionally accredited educational institution which has U.S. Department of Education approval, is preferred.
- Candidates possessing their professional clinical license (LCSW, LCPC, LMFT), or non-clinical license (LSW, LPC, AMFT),will be given preference.
- Candidates with bilingual (English/Spanish) capabilities are strongly preferred;
- Should have some familiarity with IL-DMH Rule 132 & IL-HFS Rule 140 and their general provisions & requirements pertaining to the delivery & documentation of Community Mental Health services.
- Must possess above average written and verbal communication skills.
- Must possess the ability to work independently while supporting the larger team.
- Must be highly self-motivated, disciplined, organized, and require minimal oversight.
- Must be comfortable using Microsoft Word, Outlook (email and calendars), and some familiarity working with Electronic Health/Medical Records systems is preferred.
- Must be willing to commit to working 2 evening/week to accommodate those clients requiring afterschool/after work appointments.
- H.A.S. requires all staff to work onsite.
