Position Summary
The Case Manager will intake, coordinate, and oversee individualized care plans for pediatric patients, collaborating with healthcare professionals to ensure comprehensive treatment and support for optimal well-being.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Providing Case Management services to clients including referral to outside providers as
needed.
- Preparing and maintaining records and case files, including documentation, such as clients' personal and eligibility information, services provided, narratives of client contacts, or relevant correspondence.
- Monitoring treatment progress and adjust care plans as necessary.
- Providing education and support to families regarding medical conditions and therapeutic interventions.
- Collaborating with insurance providers to ensure coverage for necessary services.
- Ensuring compliance with regulatory and legal requirements in patient care.
- Collaborating with healthcare professionals to develop comprehensive treatment strategies.
- Conferring with clients to discuss their options and goals so that programs and plans for accessing needed services can be developed.
- Monitoring and record clients' progress to ensure that goals and objectives are met.
- Performing other duties as requested.
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Bachelor's degree in nursing, social work, or related field.
- Three to five years of case management experience preferred.
- Familiarity with healthcare regulations and compliance.
- Proficiency in medical documentation and record-keeping.
- Knowledge of child development and pediatric medical conditions.
- BLS Certification (Required)
