Position title: CRU Lead LPN- I/DD
Reports to: CRU Program Director
FLSA Status: Non-Exempt
Summary: Responsible for the medical treatment, monitoring, and medication of CRU I/DD individuals. Position also provides supervision and training regarding policy and procedure to other staff.
Essential Duties and Responsibilities:

Monitor quality assurance for I/DD Individual care, performance, and documentation.
- Monitor and coordinate the supply/inventory of medication for I/DD individuals.

Coordinate with outside agencies for continuity of care of I/DD individuals.

Train nursing and AMAP staff on new processes related to the I/DD department.
- Works with nursing staff within own department and across Westbrook Health Services (WHS) departments to maximize continuity of care and efficacy of the nursing program.

Audit and coordinate the maintenance of medical charts of I/DD individuals for quality assurance.

Coordinate the monitoring of the general health condition of I/DD individuals, including vital signs, performing basic medical treatments, and dispensing medication.

Maintains the nursing and med pass schedules.

Ensure seamless communication of physician orders and changes in I/DD individual health status to include but not limited to; doctor appointments, hospitalization updates, transportation, and incident report related information (loop closer).
- Complies with federal, state, and company policies, procedures, and regulations.

Maintains confidentiality of all information and complies with HIPAA regulations.

Must have valid driver’s license and automobile insurance.

Must pass Criminal Investigation Background and DHHR background checks as well as post offer drug screen.
Education and/or experience: Current LPN state licensure without restriction. At least one year of nursing experience. Prior computer/PC experience using computerized medical records.
Physical requirement: The employee is frequently required to walk; use hands to finger, handle, or feel; and reach forward with hands and arms. The employee is occasionally required to sit and stoop, kneel, or crouch. The employee must occasionally lift and/or move up to 50 pounds.
