PROGRAM MANAGER
JOB DISCRIPTION
PRIMARY OBJECTIVE:
A Program Manager (PM) is a great leader of many, who oversees, plans, and directs the execution and tracking of programs. A PM preforms independently, at a high-level, which allows them to understand how projects fit into the organization, and more importantly how they can be best executed simultaneously, within expected compliance. These gifts allow one to align projects with the companies’ strategic objectives. A PM must excel at risk management, resource management and project planning, among other things. This role, in leadership, involves coordinating with multiple teams, project managers and key stakeholders to support organizing various programs and activities to improve company efficiencies and program accountability.
HOPE’s programs support transitions out of homelessness, transitions for other living in the community, and offer supportive services. These varied programs are designed to assistance people with flourishing in their community of choice, which should come with a “loud” voice in advocacy. The divine purpose is to advocate for those in need to PLAN, FIND, and KEEP suitable, safe, and affordable housing.

EXPECTATIONS WITHIN THE ROLE
ü Manage the day-to-day operations of programs, including organizing and administration
ü Complete quarterly reports with person served assuring integrity of the service provided
ü Oversee effective orientation and leadership development, performance evaluations
ü Perform assessments, intakes, and submit documents to DHS for program eligibility
ü Monitor allocation of units, audits, and timing to ensure appropriate and efficient utilization
ü Remain in compliance with state, federal, DHS and other licensers and certificates
ü Directly coordinate Person-Centered planning tools (TPT), intakes, and transitions
ü Implementation of program planning; monitoring delivery of services and supports; ensure services are consistent, engaging, and relevant to the individuals served
ü Provide essential transportation towards finding and/or and maintaining housing
ü Monitor and developed new program policies and procedures for best practices,
ü Advocate, research, and assess community resources for persons served
ü Maintain 3/4 billable time of documented services provided
ü Manage an assigned caseload, including the programs caseload, for efficiency
ü Completed thorough and comprehensive documentation
ü Manage programs caseloads and assign accordingly
ü Provide direct support to corporate management, and corrective action for employees
ü Making proposals on hiring, terminations, transferring, promotions, salary changes,
ü Supervise recruitment, retention, and training of new hires and current staff
ü Integrate and prepare staff to transition into Pavillio EVV system for accountability
ü Accountable for the submission of accurate bi-weekly audited billing sheets
ü Regularly audit your team work ethics for expected outcomes and/or redirection
ü Ensure accurate claims are being sent to DHS with poof of service provision (auditing)
ü Accurate data entry and updated service binders and communication lists
ü Preserve professional relationships with agencies, case managers, and stakeholders’
ü Ability to problem solve in high-intense situations toward meeting the ultimate goals
ü Establishes and oversee administrative processes; meet objectives set by management
ü Conduct Team Huddles and meetings as needed
OBLIGATIONS
ü Understanding and adhering to program rules and restrictions
ü Assessing participants eligibility and determining their housing and service needs
ü Assisting participant with efficient searches for available and suitable housing
ü Develop a customized, person-centered housing plan with participants
ü Provide support aiding in applying for benefits to increase financial independence, various skills ability, access to essential community resources, and connecting with various providers
ü Respond to client crisis as they arise and work to problem solve and deescalate situations
ü Provide community-based Case Management that includes frequent face-to-face and phone contacts with each participant, per week
ü Communicate with case management team members, community professionals, and other service providers to ensure that services for the participant are properly coordinated
PROGRAM EXPECATIONS
ü Collaborating with government agencies, community organizations, and property owners
ü Maintaining electronic health records, activity documentation, and reports to ensuring it’s in accordance with company expectations and in compliance with HIPPA and Data Privacy
ü Investigating and resolving client and landlord issues
ü Ensuring clients understand their rights and responsibilities as tenants
ü Transportation of participant in your personal, insured vehicle, as needed
ü Providing clients with information & refer them to additional support programs or services
ü Continuously exercises good judgment in handling participant issues
ü Provide and promote unconditional positive regard to all participants regardless of race, creed, gender, religion, sexual orientation, disability, and economic status
ü Participate in staff meetings and meet regularly with program coordinator
ü Maintain current knowledge of RSC & MHM requirements, and developments
ü Support with innovation and tenacity towards providing creative solutions in supporting
ü Performs other duties as requested or assigned
EDUCATION AND SKILL QUALIFICATION
ü Bachelor’s Degree or better
ü 2+ years of direct support and case management experience
ü Reliable Transportation with valid driving license and auto insurance; have a safe record
ü Ability to travel distances, as we are authorized to serve the state of Minnesota
ü The ability to work with integrity, independently
ü Knowledge of housing resources, social service agencies and community services
ü Experience with working in a diverse, continuum of services in a fast-paced, growing agency
ü Good verbal and written communication, diverse interpersonal, and active listening skills
ü Strong analytical, mediating, and negotiation skills
ü Excellent organizational and case management skills
ü Comply and retain a thorough working practice of agency policies, protocols, and procedures
ü Ability to operate Microsoft Office programs and can type 45 WPM
