Overview:

The Certified Home Health Aide (CHHA) is an important member of the care team who performs various personal care services to meet the patient’s personal needs while promoting the patient’s comfort, safety and peace of mind. The CHHA is responsible for observing patients, reporting these observations, and documenting observations and care performed. The CHHA works with the Hospice team and the patient’s caregivers to ensure excellence in care that is delivered in accordance with the patient’s wishes.
Essential Job Functions/Responsibilities
Responsibilities of the CHHA include, but are not limited to, the following:
- Providing personalized care with ADLs like transfers, dressing, bathing, eating, ambulation, and continence care.
- Communicating with patients and their caregivers in a manner that is respectful and supportive of their values and wishes.
- Changing bed linen
- Skin care to prevent breakdown
- Keeping patient’s living area clean and orderly, as appropriate
- Planning and preparing nutritious meals.
- Assisting with exercise according to the plan of care.
- Doing patient’s laundry, as appropriate.
- Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
- Reporting on patient’s condition and significant changes to the assigned nurse.
- Adhering to documentation and care procedures and standards of personal and professional conduct
- Complies with infection prevention and control policies and procedures.
- Honors patient and caregiver's rights.
Position Qualifications
- At least 18 years of age.
- Must have a Certified Home Health Aide certificate (CHHA) as well as CPR and First Aid certificate.
- Ability to read, write, speak and understand English and follow written instructions and document care given.
- Ability to travel per job requirement.
- Valid driver's license, auto insurance, reliable transportation and willingness to drive to patient locations.
- Must clear a criminal background check, physical and a drug screen.
Experience
One year of hospice care experience preferred.
Licenses & Certifications
Certified Home Health Aide License
CPR Certification

Pay: $40 - $45 per visit
