Responsibilities

Responsible for coordinating the care and service of patients across the continuum promoting quality care, effective utilization of resources and assuming a leadership role with the interdisciplinary team to achieve desired clinical and resource outcomes.

This is accomplished by assessing, planning, implementing, and evaluating the delivery of services and working collaboratively with all members of the health care team.

Plans and coordinates a comprehensive plan of care using a concurrent integrated review, standard operating procedures, case management tools, and collaborative efforts with all members of the patient’s family and/or significant other to enhance quality care and reduce costs.

Coordinates patient care to ensure appropriate and effective clinical outcomes.

Communicates with third party payors, peer review entities and physicians’ offices to gather and provide clinical and fiscal information for financial optimization.

Contributes to a positive work environment and performs other duties as assigned or directed in order to enhance the overall team efforts of the department/organization.
Qualifications
Job Requirements:

Active Registered Nurse License in state of Texas
-
3 years of hospital experience required
Preferred Job Requirements:

Previous case management experience preferred
