360 Wellness is hiring a full time Care Coordinator to join our work family. A medical case manager serves as a liaison between a patient with health issues, for the doctors and medical staff, and the patient's family and friends. While a medical care coordinator does not offer medical diagnosis or treatment, they do however monitor the patient's treatment plan to ensure that those responsible for carrying it out are doing their respective jobs properly.
Functions of the job:
- Maintain records and follow up with client funding needs.
- Ensuring that client is aware of all appointments and follow up with all outside appointments when needed.
- Provide support to patients when referred to outside services including communicating and collaborating with referring agencies and patient.
- Check voicemails/calls every morning and follow up with clients daily.
- Proficient in understanding of all funding sources. Review Care Plans with patients as well as set them up on payment plans if needed.
- Provide excellent customer service to all internal and external customers.
- Provide prompt communication within one business day to all contacts (e.g. email, phone, fax, etc.).
- Communicate with collateral supports (OH Nurse, behavioral health programs, parents, case managers, etc.) when appropriate.
- Maintain client medical files with accuracy and confidentiality.
- Document all incidents immediately and report as required.
- Other duties as assigned. Attend staff meetings as required.
Requirements
- Medical assistance or certification or similar experience preferred but not required.
- Experience in sales and finance.
- Interest in medical services, Chiropractic, Medical, and Regenerative Medicine
- Must be 21 years of age.
- Be able to maintain large amounts of patient paperwork with finance record keeping.
Compensation and Benefits
- Compensation starts at $32,000-$35,000 annually (DOE and is contingent upon experience)
