JOB SUMMARY
Responsible for planning, coordinating, and supervising the delivery of health care for CCHC health clinics. Must deal with evolving corporate health care delivery models, technological innovations, restructuring of work and workflow processes. To support and ensure comprehensive orientation of staff is achieved to maintain the highest quality patient care. To develop and implement outcomes management plans for purpose of performance improvement and enhancement of efficient clinical practices. Ensure all service lines for clinical operations are maximized to achieve exemplar patient care and experience
- Assist in the development of operational policies and procedures
- Analyze and improve operational Key Performance Indicators
- Develops and manages the implementation of new clinical processes/protocols for the health clinics; and the development and implementation of a system to track and monitor adherences to processes/protocols
- Analyzes and implements changes in the corporate clinical operations systems, policies and procedures and ensures their implementation.
- Participates in professional development activities to keep current with trends and practices in ambulatory health care, especially corporate onsite clinics.
- Meets with clinical staff regularly in the corporate health clinic locations to assess performance of clinics and determine improvement needs
- Identify improvement opportunities to ensure the highest quality care and services; and institutes and promotes continuous quality improvement processes relating to all organizational aspects of providing excellence in patient care
- Oversees the monitoring and assists in the analysis of data to identify trends and develop process improvement plans.
- Function as liaison, as needed, between physicians, non-physician providers and all clinical staff.
- Analyze and recommend any EMR and Dentrix updates with the software vendor, plan monitor and oversee the implementation of the Updates
- Identify any improvements with EMR and EDR templates, review, recommend and implement any changes, additions and deletions of templates
- Assist operations to determine what software changes are required and how they must be set up to capture data, set up modules and make template changes, train staff on new workflows
- Develop a communication strategy for providing timely and accurate information on policy, procedure and organizational processes on a regularly schedule basis.
- Responsible for the overall operations of the clinics, including all aspects of care for both medical and Dental, pediatric and adult medicines.
- Oversees clinic compliances with Federal, State and county regulations via submission of required documentation.
- Operational Grant Management, including creating budgets, writing narratives, identifying any compliance issues related to Grants
- Create a plan/checklist for consistent look and feel for all clinics
- Assisting COO with any projects, including opening new sites, expanding services
- Ensures a consistent look and feel is maintained for all clinics, which may involve future facility management
- Creating agendas for Operations Team’s monthly meetings, keeping minutes
- Partake in all expansion or renovation efforts for all clinical facilities
- Operational Project management and detailed follow up
- Stay updated on program requirements and adjust/maintain workflows for compliance
- Responsible for all reporting with CMS and other agencies as needed
- Create and change all operational policies and procedures as it relates to PCMH
- Define and create a Care Team and develop workflows for new model of care
- Create Care Team and Care Management Workflows
- Assists with empanelment efforts to ensure sustainability
- Assist with creating scripts to help providers to move away from physician centered care to new care model as defined in PCMH
- Assist with implementation of PCMH in all clinics
- Continue to foster an environment that empowers lead staff members to make decisions
- Help to maintain a culture that recognizes staff accomplishments
- Assist to develop performance improvement plans
- Assist with achieving minimum standards for support staff
- Support all efforts for continued growth of staff
- Host regular interdepartmental meetings to ensure communication and execution of improvements
- Participate in regular provider meetings to engage, inform and create an opportunity for feedback
- Collaborate with Medical Director to ensure CCHC achieves provider engagement
- Ensure that the Operations team is engaged in all conversations/and or decisions that impact clinical operations (medical, dental and mental health).
- Responsible for responding and taking any necessary actions for all patient feedback, which includes feedback received in the clinics, by phone, website, social media, the patient satisfaction survey and focus groups.
- Incorporate patient feedback into the organizations’ daily practice.
- Undertake continuous education to remain current with industry standards
- Bachelor’s Degree, or Master’s Degree College or university or related experience.
- At least 1– 3years of clinic operations experience preferred
- Demonstrated experience dealing with Electronic Medical Records (EMR)
- Demonstrated experience in the conversion of EMR and EDR
- Demonstrated experience in financial planning and analysis with previous experience, information technology and data analysis
- Skills should include organizational development, personnel management, budget and resource development, and strategic planning; demonstrated success developing and monitoring systems to manage both operational and programmatic work that involve high levels of collaboration.

- Proficient knowledge of Microsoft Outlook.
