Job Summary:
We are seeking a compassionate and experienced Home Health Aide to provide high-quality care to patients in their homes. As a Home Health Aide, you will play a vital role in assisting patients with daily living activities, medical procedures, and other essential tasks. Responsibilities:
- Provide personal care to patients, including bathing, dressing, and grooming
- Assist with meal preparation and cooking as needed
- Support patients with medication administration and medical documentation
- Assist with mobility and ambulation, including transfers and wheelchair use
- Provide emotional support and companionship to patients and their families
- Monitor patients' conditions and report any changes to their healthcare team
- Assist with light housekeeping, laundry, and other household duties
- Provide memory care and dementia care, including activities designed to stimulate memory and cognitive function
- Work with patients with long-term care needs, including those with developmental disabilities
- Demonstrate a commitment to patient-centered care and adhere to the highest standards of professionalism and confidentiality
- Collaborate with other healthcare professionals to ensure seamless care transitions
- Maintain accurate and thorough medical documentation
Skills:
- Strong understanding of patient care principles and practices
- Experience in long-term care and dementia care
- Ability to prepare meals and cook for patients
- Proficiency in medical documentation and medication administration
- Compassionate and empathetic attitude towards patients and their families
- Excellent communication and interpersonal skills
