Under low to moderate supervision and according to established nursing standards, performs professional nursing services in an office or clinic setting. Assists physician or other licensed practitioner with diagnostic and treatment procedures. Performs routine patient care and education activities. Some ancillary functions included. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patient served in assigned area.
Will be a clerical and clinical position and will perform both room and check in/registration.
Independently triages complex patients whether face to face or by telephone to appropriate levels of care. Processes clinical information to direct patients to appropriate follow-up in a timely manner.
Works collaboratively with available Mercy Resources (physician and other care providers.) to implement a plan of care that best meets the patient needs. Adapts plan of care, as possible, to fit with the patient/family insurance or financial resources.
Obtains medical histories, vital signs, weight and measurements. Prepares patient for visit, conference or procedures. Performs EKG’s, spirometry, medication administration, IV therapy and venipunctures.
Assists in obtaining specimens, biopsies and cultures in accordance with established infection control procedures.
Initiates ordering of lab work, outside testing and hospital admissions. Reviews diagnostic data and initiates follow up as appropriate.
Utilizes and maintains proficiency in use of electronic medical record.
Educates and counsels patients to be active partners in plans that promote health, prevent disease and seek early treatment.
Provides nursing case management for assigned patients. Follows guidelines for care according to diagnosis/procedure.
Functions effectively and efficiently in assigned clinics. Manages multiple tasks/demands for professional assessment, problem solving, education and counseling as well as basic nursing tasks.
Develops care guidelines for patients with special diagnoses in collaboration with healthcare team members.
Demonstrates knowledge and appropriate utilization of community resources and agencies to promote ongoing support for individual patients and the patient population served.
Is knowledgeable of and participates in performance improvement activities of the department. Actively promotes and achieves standard thresholds of the department.
May perform other clinic duties on an as needed basis. Routinely oversees ongoing clinical performance of ancillary staff. Assures that the environment is supportive of quality clinical practice (safety, infection control, pharmacy control, etc.)
Acts as resource/consultant/support to other health team members.
