Position Description
A Family Coach will assist clients in exploring their interests and abilities through a wide range of awareness and assessment activities. Family Coach will work with one-on-one with a participant in a collaborative process to help identify and achieve individual and family goals. Support clients in the development of job readiness and personal qualities to prepare them to be competitive in the job market. Coaching is responsive to family interests, as determined by the family. This means the coaching approach needs to be flexible. Coaches meet parents and families where they are, and are able to change hats as families identify their interests. Family-centered coaching supports families to move fluidly throughout the community to include utilizing linkages from private and public agencies, community groups, accessible resources with diverse backgrounds.
Family Coaches are responsible for providing one-on-one coaching sessions and case management support to participants and adhering to all CHA implementation requirements. Family Coaches will be energetic, flexible, collaborative, and proactive team members who positively and productively support whole families.
Duties
- Manage a caseload of CHA participants
- Performs intakes, assessments and self-sufficiency planning of families and individuals.
- Facilitate the development of success plans with participants based on intake and assessment outcomes
- Provide linkage to available services and advocate for additional community resources
- Monitor goals set within success plans
- Track client services and record via physical and electronic formats.
- Maintain neat and accurate case files for program participants (according to CHA and funder standards)
- Establishes and develops relationships with applicable agencies to create partnerships for referrals. Coordinate community services including, employment training, and continuing education programs, referrals to shelters, alcohol and drug treatment services, etc.
- Develop a relationship with providers
- Ensure case file maintenance and access to program case files as determined by Quality Assurance and/or contract guidelines. Administer required program survey tools in accordance with Quality Assurance and contract directives.
- Identify and establish appropriate linkages with education, health, and community service providers as resource options for program population.
- Maintain the integrity of all case files for the 2-Gen Program to include opening, closing, and filing of case documentation.
Knowledge, Skills and Abilities
- Must be aware of the CHA Plan for Transformation and overall knowledge of the FamilyWorks program. The functions and resources of public and private social welfare and related agencies providing services to individuals within the respective area of specialization, current social, economic, environmental and health problems affecting the clients’ social and environmental factors which result in the need for resources, placement or institutionalization of individuals.
- Knowledge of funder requirements
- Excellent communicator, verbal and written, to individuals and groups.
- Must have moderate proficiency in keyboarding or willingness to receive additional training as needed to improve speed and accuracy.
- Ability to communicate effectively and project a professional image when giving and taking information in writing, in person, and over the phone.
- Must be able to work in and independent manner.
- Ability to take initiative and prioritize tasks; good time-management, problem-prevention, and problem-solving skills.
- Ability to work accurately with close attention to detail.

Qualifications
- Bachelor’s degree social work, sociology, psychology, human service administration or related field
- Excellent written and verbal communications skills
- Excellent organizational skills
- 1-2 years’ experience performing case management, counseling, and/or coaching in a social service setting
- Knowledge of local ecosystem of support service providers
