Primary Responsibilities include:
- Assess and reassess the physical, environmental and psychosocial needs of clients in their home setting
- Maintain continuity of care by coordinating with clinical manager, home health aides, physical therapy team, primary care physicians and client's caregiver(s)
- Document interventions, observations and evaluations using OASIS and Axxess Home Care Software within required time frame
- Create individualized Plans of Care for new and existing clients
- Conduct supervisory visits of Home Health Aides in accordance with State and Federal mandates
- Participate in orientation, on-going education programs and ensure successful completion of competency requirements
- Ensure that all home care provided is compliant with State and Federal regulations and Joint Commission standards
- Provide services that are congruent with the policies of Independence Health Services and within the scope of professional nursing practice as defined in the Nurse Practice Act

REQUIREMENTS:
- Current RI nursing license
- Current CPR certification
- Current Driver's license
- Minimum of 1 year experience as an RN
- Able to manage time effectively and work independently
