The primary responsibility of the Nurse is to foster the patient experience, improve outcomes, and promote health and wellness/quality of life for our Village patients. The RN will lead our team in a positive and productive environment, modeling exemplary patient care.
Nurses plan and coordinate primary care services to meet the needs of individuals and families within their visit location and communities under the supervision of the provider; with emphasis on community health education/experience. In addition, the RN will coordinate the care and education activities conducted by health care professionals to help patients with chronic diseases and health conditions such as diabetes, high blood pressure, etc. in order to maximize understand of their condition and live successfully within the scope of a nurse. A Village nurse has a strong sense of self, compassion, dedication, and the ability to assess and provide skilled care in an autonomous environment, to include patient’s home, living facility, or clinic. It is the goal of Village to have the RN actively participate in a strong interdisciplinary team, in collaboration with the provider, to make an impact in patient care and grow in your nursing career.
Essential Duties and Responsibilities (but not limited to):
1. Provides telephonic triage or requested support and / or virtual monitoring. Offers subsequent recommendations, education or care advice using decision making tools, critical thinking/clinical judgement and defined workflows.
2. Participates in care coordination, by partnering with the customer, to reduce readmissions, enhance chronic disease management, and manage health risks to including scheduling provider appointment and provider communication based on patient needs.
3. Ensures accurate, timely documentation in the EMR (including scanning) according to best practice, guidelines, or workflow policy.
4. Participates in virtual monitoring (RPM) and subsequent reporting/ escalation to support services identified by customer.
5. Provides additional support to VPCP business functions, as identified, to ensure all patient needs are being met and continuity of VPCP care is maintained.
6. Participates in process improvement, professional development, peer development, peer review and shared governance.
7. Performs Medication reconciliation per practice standards set by VPCP.
8. Coordinates chronic care management for assigned patients, recognizes needs for chronic condition care planning, and develops/implements nursing plans of care in collaboration with Provider(s).
9. Performs skills consistent with nursing scope as required with proficiency and identifies areas of growth, seeking learning and mastery opportunities in order to deliver exceptional and compassionate care – see list of skill requirements.
10. Develops a level of professional nursing practice and accountability that is consistent with the successful VPCP mission, code of conduct, and standards of practice identified by VPCP that are within the scope of a registered nurse.

Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required. Good driving record.
Physical Requirements and Working Conditions:
Must sit, stand, walk, lift, squat, bend, twist, crawl, kneel, climb, and reach above shoulders throughout the workday. Drive to and from patients place of service up to eight hours per day.
Must be able to:
lift up to 50 lbs from floor to waist.
lift up to 20 lbs over the head.
carry up to 40 lbs a reasonable distance.
Full range body motion including handling/lifting patients
Ability to drive up to 150 miles daily in urban and/or rural settings (company vehicle provided)
Ability to read and understand road maps
Ability to sit or stand for long periods of time
Must be able to:
push/pull with 30 lbs of force.
perform a sliding transfer of 150 lbs with a second person present.
Will be exposed to the following hazards on a frequent basis: mechanical, electrical, chemical, blood and body fluids. Will be required to wear protective clothing and equipment as needed.
Must have functional speech, vision, hearing, and touch with ability to use fine-hand manipulation skills.
Operates all equipment necessary to perform the job.
